Patient Direct Payment
In evaluating the medical practice management strategies, need to take into consideration the difficulties may be facing in collecting money from the private-pay patients. With out of pocket costs increasing and higher unemployment, people still need care – and to succeed, need to be able to manage their need for care versus the need to make a living. Educate the patients about what they will be responsible for up front. Don’t let the patients be surprised by a bill in the mail have the staff let them know before they see what their insurance will and won’t pay and what they are going to owe. Then give them an opportunity to pay or set up a payment plan immediately. Set up payment plans so the patients can manage their health care costs as a regular part of their budgets. Working with patients can increase the amount thee paid as patients realize there doing the best for them. If have someone who chronically misses payments or doesn’t pay, consider requiring that patient to pay the practice up front before receiving treatment. Once set up a payment plan between the patient and the practice, give the patient a coupon book rather than sending statements. This will decrease the operating costs and provide the patient with a physical reminder of their debt. Collect fees up front as much as possibly can, including known co-pays and fees for elective visits. Create a Web site that has a shopping cart feature that patients can use to pay the practice online. Consider instituting an online debiting system using patient’s credit cards or bank accounts. Set up a subscription plan with patients that allow them to pre-pay for services. Guarantee a discounted price on select services for patients who join the plan. Accept cash only from patients who have bounced a check with the practice or included their debt to the practice in a bankruptcy. Most physicians don’t like having to think about the possibility that they won’t be able to treat any patients just because they lack the means to pay. But need to have good medical practice management strategies around payment to keep the practice in business. With potentially shrinking reimbursements from the government and private insurers, the practice may become increasingly dependent on private pay dollars. Work with the patients and educate them up front, collecting that money won’t be such a difficult accomplishment. Some physicians have had such an easy time managing private-pay patients that they’ve gone to a cash-only model and shut out private and public insurers alike. This management practice certainly cuts overhead costs while giving the doctor more autonomy in treating patients. Choose to manage the private pay patients, need to institute good medical practice management strategies now to avoid losses and frustrations later.
Medical Pedicure
A medical pedicure provides a safe and clean environment to have your nails and skin of the feet treated. Medically based corrective pedicure occur under sterile conditions. A pedicure is a way to improve the appearance of the feet and toenails. Medical pedicures are similar to manicures. Pedicures are not just limited to nails; they involve the removal of dead skin cells on the bottom of feet using a rough pumice stone. Additionally, leg care below the knee is a common and now expected service included in pedicures. Leg care includes depilation via either shaving or waxing followed by granular exfoliation, application of moisturizing creams and a brief leg massage. Medically based corrective pedicures occur under sterile conditions with the supervision of a physician. The physician begins with an anti-bacterial solution and gently exfoliates the feet to eliminate dry skin and calluses. The treatment includes detoxifying, nail shaping and reflexology to improve the look and feel of the feet. This treatment finishes with a matte nail buff or polish. Medical pedicures are available to treat conditions of the nails and the skin of the feet and legs. In a safe and comfortable setting, a Certified Pedicurist who is a licensed aesthetician will use sterile instruments to undertake a basic medical pedicure, dry sole treatment, nail fungal or nail repair treatment or a foot facial. Manicures have been popular for 4,000 years. In southern Babylonia, noblemen used solid gold tools to give themselves manicures and pedicures. The use of fingernail polish started even earlier. Originating in China in 3000 BC, nail color indicated social status. According to a Ming Dynasty manuscript, royalty painted their fingernails black and red. Ancient Egyptians had manicures dating all the way back to 2300 BC. A carving in a pharaoh's tomb depicts early manicures and pedicures, and the Egyptians paid special attention to their feet and legs. The Egyptians also colored their nails, using red to indicate the highest social class.

FDA Medical Device Registration
Owners or operators of places of business that are involved in the production and distribution of medical devices intended for use in the United States must register annually with the FDA, which is a process known as establishment registration. Congress has authorized FDA to collect an annual establishment registration fee for device establishment registrations. A detailed list of all those establishment types that have to pay the registration fee is available at "Who Must Register, List and Pay the Fee." There are no reductions in annual establishment registration fees for small businesses or any other group. Most establishments that are required to register with the FDA are also required to list the devices made there and the functions of those devices. If a device requires pre-market approval or notification before marketed in the U.S., then the owner/operator should also submit the FDA pre-market submission number. The amendments to the Medical Device User Fee Modernization Act require that after September 30, 2007, all businesses submit registration and listing information electronically. Registration and listing provides the FDA with the location of medical device establishments and the devices manufactured at those establishments. This information augments the ability of the United States to prepare for and respond to public health emergencies.
